We require ALL this information be completed in full, and on file, for players to be eligible for games. Please ensure all information and documents are complete and you have reviewed all policies (online or in print).
Forms are available to complete online or in print. When you have completed these forms, you can email a copy to our registrar, or print and bring to one of our registration evenings in the spring.
If you are registering after spring/fall season has started, please bring your completed registration forms to the field with you. Players will be suited up while you finish the registration process with one of our Executive members.
